Outpatient referrals

We evaluate most new patients on an outpatient basis.

Submit this form along with the patient's records via:

• Fax 323-984-9111, or
• Email referrals@gynoncspecialists.com

For HMO patients, please include the insurance authorization.

Once you’ve contacted us, we will notify you or your patient to arrange an appointment within 3 business days.

You and your patient may be asked to provide additional information to ensure a comprehensive consultation. After the evaluation is complete, you’ll receive a written report of the findings and recommendations.
